Job Description:

Job Summary: Experienced RN-Case Manager sought to support patient discharge planning and identify barriers to safe hospital discharge in a collaborative team environment.

We are seeking a skilled RN-Case Manager to join our team at SSM Health St. Joseph Hospital - St. Charles. This individual will float to our 6 St. Louis locations, assessing and developing discharge plans for patients to ensure safe transition to next level of care.

Key Responsibilities:


  • Ensure compliance with Condition of Participation (CoP) discharge planning.
  • Identify and escalate gaps/barriers related to inpatient and observation length of stay.
  • Assess patients at high risk for readmission, identify root cause, and develop plan for discharge to next level of care.
  • Participate in multidisciplinary team to improve standard work and identify opportunities for improvement.
  • May be responsible for utilization management process, ED case management process, admission process, and liaison with post-acute providers.
  • Apply evidence-based practice and scientific knowledge to nursing practice, ensuring patient care is delivered based on age-specific needs and clinical needs.
  • Work in a constant state of alertness and safe manner.
  • Perform other duties as assigned.


    Requirements:

    • Graduate of accredited school of nursing or education equivalency for licensing.
    • Three years' acute care experience.


      Physical Requirements:

      • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
      • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
      • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
      • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
      • Frequent keyboard use/data entry.
      • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
      • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
      • Rare climbing.


        Professional Licenses and Certifications:

        • Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
        • Registered Nurse (RN) Issued by Compact State.
        • Registered Nurse (RN) - Missouri Division of Professional Registration.
        • Registered Nurse (RN) - Oklahoma Board of Nursing (OBN).
        • Registered Nurse (RN) - Wisconsin Department of Safety and Professional Services.


          Work Schedule: PRN / Per Diem Shift (United States of America)
